Supply Reimbursement Form

The OERB will reimburse you for your out-of-pocket expenses associated with curriculum materials. Just follow these steps.

Teach the OERB curriculum this school year
Save your receipts for Little Bits, Fossils to Fuel, Fossils to Fuel 2, Petro Active, Rockin’ Ratios, or CORE Energy consumable materials
Complete and submit this form and attach your receipts

**Please note that we can only reimburse you for materials listed in the OERB curriculum guides. Along with providing receipts for the consumable materials, you must list the materials you used, and the cycle you used them for. You must complete this form in order to be reimbursed. If you are unsure about approved materials, please call 1.800.664.1301 ext. 236 or email teachers@oerb.com with questions.
